Welcome to an opportunity to become more healthy! The Corporate Cup "Healthy Team" event is designed to promote your healthy lifestyle changes while allowing your company to receive recognition for encouraging these changes. Over 60 teams participated in last year's event and have completed the required pre-testing. Both a pre and a post-test are required for participation in this event and testing dates are listed above. The day and time you pick for your pre-testing will generally be the same for Post-testing.

The goal is for each team of two (2) people to accumulate a total of 100 points or more. Healthy meals are scheduled throughout the competition and reservations may be made by clicking here.

GOAL: To offer a Corporate Cup event which will promote healthy lifestyle changes and allow companies to receive recognition for encouraging these changes.

BENEFIT: EACH HEALTHY TEAM PARTICIPANT RECEIVES A THREE (3) MONTH MEMBERSHIP ($164 VALUE) TO THE VALLEY YMCA DURING DURATION OF HEALTHY TEAM! Participants are also eligible (and required) to participate in the free healthy meals program.

GUIDELINES: Each company will choose the desired number of two (2) person teams to participate in this event. Points will be awarded for certain healthy behaviors and for measured healthy physiological changes and additional healthy events scheduled throughout three (3)-month period. The key to choosing the participants for this event is to find people who are highly motivated to make some healthy lifestyle changes. There are six (6) requirements for participation in this event.

1. A written physician's consent will be required for each participant in the event (PRIOR TO INITIAL SCREENING). YOU CANNOT BE TESTED WITHOUT YOUR CONSENT FORM. IF YOU SHOW UP FOR PRE-TESTING WITHOUT PHYSICIAN CONSENT FORM. YOU CANNOT BE TESTED!!!!!!! (Click here to print the form)

2. Each participant must go through an initial health screening and a final health screening. (It is recommended that you refrain from eating one hour prior to testing.)

3. Each "HEALTHY TEAM" must participate in at least two (2) Corporate Cup events.

 
4. Each person must exercise three (3) times weekly for at least 30 minutes. (Exercising at the Y is not mandatory - other facilities may be used.)

5. Each person must maintain a food diary for at least three (3) consecutive days each month for the duration of the event.

6. Each person must complete a Participant Contract form, to be turned in at Post-testing.


HEALTH SCREENING (REQUIRED): All participants in this event will participate in an initial and final health screening which will consist of blood pressure, weight, measurement of percent body fat, blood cholesterol screening, and a step-test (a test consisting of stepping up and down on bench to determine cardiovascular fitness) and participate in at least one (I) healthy meal at Griffin Hospital. Details and dates are included on the web site. Points will be awarded for improvements in these areas. Extra points will also be awarded for other additional lifestyle modifications.

Health Risk Assessment: Take advantage of a unique opportunity to participate in Project Co-N -E-C-T, a Valley-wide effort to assess the community's Need for Evaluation, (primary) care and Treatment. This health risk assessment (HRA) is designed to help you improve your overall health and well-being. After completing the HRA questionnaire, you will be provided with a personalized index of your perceived and real wellness along with recommendations on how to lower your risk for preventable illness or injury. If you have completed this previously, there is no need to do it again For details or an appointment, call (735-3942).

MISCELLANEOUS (OPTIONAL) 1. Points will be awarded to each team that consists of one (1) male and one (1) female. 2. Points will be awarded for each person with a valid Red Cross First Aid Certification, current CPR or BLS (A,B,C or D) card. (Attach photo copies).
