Welcome to an opportunity to become more healthy! The Corporate Cup "Healthy Team" event is - designed to promote your healthy lifestyle changes while allowing your company to receive recognition for encouraging these changes.
There is one thing you must make arrangements to do immediately. In order to participate in this event you must have a physician's release. This release must be completed, signed and brought with you to the pre-test.
Please have all forms filled out prior to arriving for the pre-test on March 4 or 6, 1998.
Pre-testing will be done through the Griffin Hospital Cardiac Rehabilitation Dept., 130 Division St., Derby. Dates and times to follow.
Please call Valley YMCA, Sharon Holly, for appointment for pre-testing and post-testing at least two (2) weeks prior to the pre-test date. Shirts will be distributed at pre-test. You will be required to wear the issued shirt at both pre-test and post-test.
Testing (both pre and post) is a requirement for participation in this event and you must be tested on those dates. Exceptions to this are made only in extreme circumstances and only by pre arrangement, so please call 736-1435 immediately if either of these dates is impossible for you to make. The decision to allow alternate testing is made on an individual basis. The day and time vou pick for pre-testing in March will be the same for Post-testing in May
The rules for accumulating points toward the healthy team event are explained in the accompanying packet, so take time to familiarize yourself with the scoring system. The goal is for each team of two (2) people to accumulate a total of 100 points or more.
GOAL: To offer a Corporate Cup event which will promote healthy lifestyle changes and allow companies to receive recognition for encouraging these changes.
BENEFIT: EACH HEALTHY TEAM PARTICIPANT RECEIVES A THREE (3) MONTH MEMBERSHIP ($159 VALUE) TO THE VALLEY YMCA DURING DURATION OF HEALTHY TEAM!
GUIDELINES: Each company will choose the desired number of two (2) person teams to participate in this event. Points will be awarded for certain healthy behaviors and for measured healthy physiological changes and additional healthy events scheduled throughout three (3)-month period. The key to choosing the participants for this event is to find people who are highly motivated to make some healthy lifestyle changes. There are six (6) requirements for participation in this event.
HEALTH SCREENING (REQUIRED): All participants in this event will participate in an initial and final health screening which will consist of blood pressure, weight, measurement of percent body fat, blood cholesterol screening, and a step-test (a test consisting of stepping up and down on bench to determine cardiovascular fitness) and participate in at least one (I) healthy meal and a stress management program at Griffin Hospital. Details and dates are included in this packet. Points will be awarded for improvements in these areas. Extra points will also be awarded for other additional lifestyle modifications.
Health Risk Assessment: (March - May) Join hundreds of others in the Valley who have taken advantage of a unique opportunity to participate in Project Co-N -E-C-T, a Valley-wide effort to assess the community's Need for Evaluation, (primary) care and Treatment. This health risk assessment (HRA) is designed to help you improve your overall health and well-being. After completing the HRA questionnaire, you will be provided with a personalized index of your perceived and real wellness along with recommendations on how to lower your risk for preventable illness or injury. MISCELLANEOUS (OPTIONAL) 1. Points will be awarded to each team that consist of one (1) male and one (1) female. 2. Points will be awarded for each person with a valid Red Cross First Aid Certification, current CPR or BLS (A,B,C or D) card. (Attach photo copies).
